Mobile Health Program

FDIHB, Inc.’s Mobile Health Program is dedicated to improving community health and well-being by increasing access to safe, quality and preventive health services.

Working with local partners at chapters, senior centers, markets, schools, and other community resources, the Mobile Health Program aims to reduce barriers to care, such as lack of transportation, trust in the traditional healthcare system, or lack of time or ability to travel the often long distances to either of our two facilities, Tsehootsooi Medical Center (Fort Defiance, AZ) and Nahata Dziil Health Center (Sanders, AZ).

In order to reach those who need services the most, the mobile units travel across Fort Defiance Service area, spanning 16 chapters in Arizona and New Mexico, to bring a range of services directly to our community members.

We aim to be a community resource for care, especially for those who do not currently have a primary care provider. Appointments and walk-ins welcome.

Services

Elder Wellness Clinic (1/month)

On the 2nd Wednesday of each month, this clinic travels to various chapter houses and provides a range of screening and education services to older community members, including eye health checks, hearing screening, immunizations against flu, shingles, and pneumonia, balance assessments and fall prevention, traditional healing, oral health education and screening, primary care visits, and medication reconciliation.

Family Practice Clinics

Staffed by a Family Nurse Practitioner or Physician Associate and a Certified Medical Assistant, this clinic is open Mondays through Thursdays at rotating community locations. We serve men and women, and children ages 3 and up. Services include: primary care, diabetes screening and management, flu shots and other immunizations, physicals, treatment of minor illness, HIV/STI testing, and more.

Sports and School Physicals: Providing a full range of services at select chapter locations to complete school and sports physical requirements; staffed by Pediatrics, Dental, Optometry, and Rehab.

Teen Wellness Clinic (2/month)

Parking at the Window Rock High School typically on Mondays, this clinic is operated by staff from Mobile Health Program to provide students with comprehensive services including: physical exams, health education, immunizations, reproductive health, treatment of minor illness, and mental health screening and referrals. We aim to be a resource for students to discuss any questions or concerns in a safe and confidential space, and to educate about and inspire careers in healthcare.
